Heparins: 
Drug: Unfractionated heparin (UFH) 
Laboratory tests: activated partial thromboplastin time (aPTT) 
Reversal Agents: Protamine 

Drug: Low-molecular-weight heparin (LMWH) 
Laboratory tests: None required, but anti–factor Xa assay can monitor levels 
Reversal Agents: Partially reversed by protamine.

Pentasaccharide: 
Drug: Fondaparinux 
Laboratory tests: None required, but anti–factor Xa assay can monitor levels 
Reversal Agents: None 

Drug: Dabigatran 
Laboratory tests: None required 
Reversal Agents: Idarucizumab, dialysis may remove drug. 

Factor Xa inhibitors: 
Drug: Rivaroxaban, apixaban 
Laboratory tests: None required 
Reversal Agents: None
